This category of visa caters for people wishing to stay in Spain for a period maximum 3 months with a purpose of medical treatment.

Applicants applying at the Spanish visa application Centre shall be levied a service charge of 755.93 DOP /- (inclusive of service tax 15% & education cess) payable in cash. Children under 6 are exempted from service charge.
Diagnostic or treatment appointment with a licensed medicinal doctor in Spain.
If the treatment will be performed in a public hospital, an authorization of the competent body in the matter (INSALUD or Consejería de Salud de la Comunidad Autónoma (names of medical organizations in Spain)) indicating the sanitary center in which the medical treatment will be performed and approximate duration thereof. To this document must be added the documents which accredit and prove the availability of sufficient economical means to be able to cover the expenses of the trip and the stay as well as the cost of the medical treatment, for the patient as well as the person accompanying. In this case it will need to be indicated whether treatment for this illness is not available within the Dominican Republic.
If the treatment will be performed in a private sanitary center, a certification of this center will need to be provided indicating the approximate duration and cost of the treatment. To this document must be added the documents which accredit and prove the availability of sufficient economical means to be able to cover the expenses of the trip and the stay as well as the cost of the medical treatment, for the patient as well as the person accompanying. In this case at least 20% of the total amount of the treatment, consultation or intervention will need to be accredited or proven.
The Consulate reserves the right to demand any additional document it may require.

Minors travelling without their parents (Minors of less than 18 years of age):
All the documents supplied by the applicant must be translated into Spanish – except the photocopies of the passports, bank statements (bank certificate with banking activities on the account for the last SIX months) and medical insurance.
The visa application and the pick-up of the passport will be done personally.
Obtaining a visa is not a right of the applicant, whichever disbursement made related to the requisites of documents and certificates demanded for the purpose of obtaining a visa are not conditions for expedition by the Consulate.
The consulate will realize the evaluation of the visa with the documentation provided, so it is in the interest of the applicant to supply all documentation which will facilitate its concession.
The response whether concession of the visa was given or not will be notified to the applicant 10 (working) days after the presentation of the application.
The consulate might request from the applicant whichever additional document it may consider necessary in order to make the decision of whether or not to approve the visa.
All the documents supplied to the consulate at the time of application for visa will not be returned to the applicant at a later time. Please submit copies of important documents, but bring the originals to the appointment for revision.

Applications accepted at the Spain Visa Application Centre will be submitted on the next working day at the Embassy of Spain/Consulate General of Spain. Once the application has been submitted, the applicant can be further requested additional documentation in support of the same. Please note as per the instructions received from Embassy of Spain minimum processing time is 15 Calendar days (It may delay depending on case to case).
All applications must be submitted at least 15 days prior to the proposed date of travel to accommodate any unforeseen processing delays.
Kindly note if application submitted in Satellite centre, it may take up to 48-72 hours in transit to Santo Domingo. All the applications received from Satellite centers will be sent to Embassy/Consulate on next working day
Processing time will start excluding the date of submission of application at BLS International Services.
Applicants submitting the applications towards the Embassy of Spain, Santo Domingo with short processing time will be required to fill up the Declaration form.
All Application submitted at visa application centre should have English/Spanish Application forms only.
Please note W.E.F 01 April 13 , Embassy of Spain has instructed that Sworn Declaration is mandatory to be provided along Visa Application form at the time of submission.
